Position Summary
Provides therapeutic interventions in the rehabilitation of patients and other clients through the use of treatment procedures and modalities consistent with the education of this specialty. Uses techniques and activities to promote the development of maximum potential of adaptation and productivity of the individual within his/her environment.
Job Description
A. PHYSICAL THERAPY
Duties: Accurately assess the needs of all individuals prescriptively referred for evaluation and/or treatment. Plan and carry out an individual progressive treatment program intended to maintain or improve each person’s physical and/or psychosocial well-being or to prevent disability within the dictates of the prescription, therapy goals and policies of the facility.
Accurately and regularly record all pertinent information regarding the Physical Therapy treatment program. Document the need for the skilled service in each record entry. Regularly monitor the RUG minutes, adjust daily within the designated minutes or adjust with other disciplines to insure the RUG is met.
Consistently performs at the required productivity level. Provide consultation to nursing service in the area of ambulation and/or transfers and the use of adaptive equipment for reinforcement and maintenance of skill.
Provide in-service programs within professional discipline for the facility when reasonably requested. Attend and contribute to patient staffing and utilization reviews held in the facility to coordinate a total patient plan of care.
B. SAFETY
Duties: Comply with all facility policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts, and personal protective equipment.
C. RESIDENTS’ RIGHTS AND POSITIVE RELATIONSHIPS
Duties: Understand, comply with and promote all rules and regulations regarding residents’ rights; promote positive relationships with residents, visitors, and regulators, to include presenting a professional appearance.
Maintain; promote established policies to ensure confidentiality of all resident information. Establish and disseminate and adhere to, HIPPA compliance policies and procedures.

D. ETHICS AND COMPLIANCE
Duties: Exhibits expected ethical and legal standards. Reinforces through examples and where applicable, conducts training on the expectation that all employees conform to these ethical and legal standards, including matters related to resident abuse or neglect.
When in doubt about whether a particular activity is proper, refrains from the activity and obtains guidance from the Director of Nursing or the Administrator.
Responds promptly and thoroughly when problems are reported. Assist in planning, implementing, and monitoring corrective action, when problems including ethics, compliance resident abuse, neglect are identified.
Maintains compliance in promoting patient privacy, HIPPA regulations and confidentiality in the delivery of care. Safeguards patients’ medical, financial, and personal information.

Qualifications
· Must have a Minimum of Doctorate degree in Physical Therapy from an accredited college or university. Registered by the American Physical Therapy Association, Inc., as designated by the initials D.P.T.
· Must hold and maintain licensed to practice in the appropriate State.
· One year of experience, in a skilled nursing facility is desired.
· Must have the ability to read, write and follow oral and written directions at a level necessary to accomplish this job.
· Must meet all local health regulations, and pass post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry.
· Must have the ability to relate and motivate patients for productivity.
· Must be able to cooperate with patients, families, peers, healthcare professionals and other staff.
· Must be observant and accurate in recognizing and documenting patient change.
· Must be creative in the development of Physical Therapy treatment programs, use of modalities and application of adaptive devices.
· Flexible in adaptation to patient or environmental change.
